Activity Centre Near Miss / Incident / Safety Issue Form

Please identify the severity of the incident based on the definitions below. If you believe the incident to be Critical or you require immediate support please contact The Scouts Safety Team on 0208 433 7100.

Significant event

• Unexpected event such as loss/disruption of services, damage to building or property or attendance by an emergency service but not involving injured people.

Basic first aid

• Where first aid treatment was given onsite

Reportable incident

• An incident requiring medical intervention but not covered in as major or critical

Major incident

• An injury resulting in an overnight stay in hospital (one or two nights)
• Multiple casualties with reportable injuries
• Collapse of structure – tents, temporary gateways, pioneering projects
• Fatality not cause by a Scouting activity – heart attack, stroke, other condition, whether known about or not

Critical incident

• The injured person is likely to have a hospital stay of 3 days or more
• Multiple casualties from the same incident with major injuries with a stay overnight for one night or more.
